prescription

Urokinase, a thrombolytic enzyme, is prescribed for deep vein thrombosis, heart attack, pulmonary embolism, and peripheral vascular occlusion, dissolving blood clots.

risk factors

Not recommended for patients with:

  • Active internal bleeding
  • History of stroke
  • Recent (within 2 months) trauma, including brain and spinal cord surgery
  • Brain aneurysm or tumor
  • Known bleeding disorder
  • Severe uncontrolled hypertension
  • Allergy to the drug

side effects

  • Most common: Severe bleeding.
  • Heart: Heart attack, recurrent blood clots in the lungs, stroke.
  • Blood: Decreased red blood cells and platelets.
  • Miscellaneous: Excessive sweating.
  • Potentially Fatal: Severe bleeding, severe allergic reactions, cholesterol embolization. Other side effects, including cardiac arrest and irregular heart rate, have also been observed in patients using urokinase.

dosage

For adults:

  • Deep vein thrombosis; Pulmonary embolism: Initial dose of 4,400 units/kg over 10 minutes, then 4,400 units/kg/hr for 12-24 hours (deep vein thrombosis) or 12 hours (pulmonary embolism).
  • Peripheral arterial thromboembolism (using a 2,000 units/ml solution): Administer via catheter directly into the clot at a rate of 4,000 units/minute for 2 hours.
  • Heart Attack: Administer 250,000 IU over 10 minutes followed by 500,000 IU over the next hour.

how to take

It is available as a solution for injection, administered by a healthcare provider into the vein.

warning

  • Avoid vascular punctures or invasive procedures.
  • Exercise caution in patients with peptic ulcer disease, bleeding disorders, high blood pressure, heart infection, recent biopsy or surgery, recent injury, or allergies.
  • Use with care to prevent injury, especially in patients with liver or kidney disease, coagulation defects, brain disease, diabetic retinal damage, pregnancy, recent surgery, childbirth, or trauma.
  • Monitor for reperfusion heart rate during heart attack treatment.
  • Increased risk of cerebral bleeding may occur.
